Library Card Application

To check out library materials, you need a Lake County library card.  If you do not have a library card, you may come to the library and request an application or you may print the form posted on this website and fill the form out at home.  Please remember to bring in some proof of residency (driver's license, water bill, electric bill, or telephone bill) along with your application.
